The first stage in becoming a Krav Maga instructor is to express your interest by submitting the form below (Please read the 'Terms & Agreements' Below the form before you submit it). Your application will then be reviewed and your suitability considered after which there will be a phone interview with the systems founder, Moni Aizik. All decisions as to who and who is not accepted for instructor training resides with him.

By Submitting This Form I Understand & Agree To The Following:

1. This certification course is taught exclusively by the Founder and Chief Instructor Moni Aizik.

2. Upon completion of the course and passing the test, I will be certified as a Combat Survival Commando Krav Maga (CSCKM) Level 2 instructor.

3. As a Certified CSCKM Instructor, I will enjoy the following benefits:

4. Should I fail, I will be allowed to attend, free of charge, any future CSCKM Intensive Level 1&2 Instructor Certification course and be re-tested.

5. Combat Survival Commando Krav Maga will not charge any licensing fees. The only requirement is that I must progress to the next level within 1 year (i.e. As a Level 2 instructor I will be licensed to teach CSCKM for 1 year. In order to keep my license valid, I will need to progress to Level 3 within a year).

6. I have no criminal record.
